National Health Plan Coverage
Fulgent and subsidiaries are in contract with over 360 national payers, covering millions of patients in the United States for genetic and pathology testing. We will submit claims for insured patients directly to insurance companies, including Medicare and Medicaid, in accordance with applicable insurer requirements. Please verify and update patient insurance information at the time of patient specimen submission and know which testing laboratories are in-network or participating providers for your benefit plan. This information may impact your level of coverage. However, regardless of your in/out network status, we are committed to making testing affordable for everyone. Patient Bills
On occasion, a patient's insurance will not cover the full amount of the bill. This may happen when a patient has not met their annual deductible, or if the service is not covered under the patient's policy. Certain tests are not covered under some Medicare, Medicaid, or commercial group insurance contracts. Compassionate Care of Financial Assistance Program
Fulgent and subsidiaries are committed to working with patients to make testing affordable. Eligibility for financial assistance is based on total income, number of household members, and additional criteria as provided in Federal and State guidelines. Patient Self-Pay
We offer cash pricing at a discounted rate to make our tests accessible to all patients. Patients who do not meet insurance coverage policies for testing or have no insurance coverage can choose the Self-Pay option to get their tests done with Fulgent.
To take advantage of the self-pay price, select the "Self-Pay" option on our test requisition form and provide the patient's contact information. If ordering online, select the "Self-Pay" option in the billing portion of the ordering process. Fulgent or one of its subsidiaries will follow up with the patient directly regarding payment.
